At Connect and Thrive, we believe that assessments are a crucial part of the therapy process. Our initial assessments and reports include a combination of formal and informal assessment tools, including clinical observations, a parent consultation and information gathering through online forms and questionnaires. Following the initial assessment, the assessment findings are put into an initial assessment report. From here, a therapy plan is created in collaboration with the family based on the identified support areas and goals from the assessment.

Connect and Thrive is a mobile therapy service. All initial assessments are required to be conducted in the client’s home environment. Following the initial consultation, ongoing Occupational Therapy sessions can be provided in the home environment if it aligns with the client’s therapy goals.
